AI innovators work to close the senior care gap in rural America

A national initiative is transforming senior care: Artificial intelligence aging-in-place platforms that support rural value-based health care initiatives.

Rural communities don’t need more reactive healthcare—they need proactive infrastructure.”

LOS GATOS, CA, UNITED STATES, April 3, 2026 /EINPresswire.com/ -- Rural America is aging faster than urban regions, yet access to coordinated home-based care is not only limited, it’s increasingly challenged.

People who request Home- and Community-Based Services (HCBS), including older adults who need help with everyday tasks like bathing, wait an average of 40 months—over three years—to get services, according to a report from KFF. Federal funding from the American Rescue Plan Act (ARPA) that supports these programs ends in December 2026.

There’s also a staffing and provider crisis: There aren’t enough trained professionals available to provide care. Because so many immigrants provide care—28% of the long-term care workforce—federal immigration policies that force people to leave, many believe, aren’t helping to resolve the crisis. Rural hospital closures and long travel distances also make care less accessible.

And while services and workforces are stretched thin, Americans are aging rapidly. The percentage of older Americans in rural communities is higher than the percentage of older Americans in urban settings. 70 percent of people will need some kind of long-term care in their lives.

The AscendRural Accelerator Program is supporting innovation to help address these issues. Zemplee, Inc., an AI-powered aging-in-place monitoring platform, just announced its selection into the accelerator. “Innovation in rural health must be both scalable and grounded in real-world constraints,” said Melissa Kjolsing, Managing Director of AscendRural. “Zemplee’s AI-enabled monitoring platform aligns with our mission to boost rural well-being, including rural care infrastructure and support sustainable, community-based solutions for older adults.”

Zemplee’s system uses ambient AI sensors placed around the home—the bed, chairs, the bathroom, the kitchen, and other places—to monitor activity and health. There are no cameras, to preserve privacy and dignity. There are no wearables—the system doesn’t depend on an older adult to remember to put on or charge a device.

Zemplee’s remote monitoring tracks movement. Depending on the system setup, Zemplee can also monitor vital signs like blood pressure. The system sends notifications to family members or professional caregivers in real time. Through passive, privacy-preserving AI monitoring and predictive analytics, Zemplee enables earlier detection of functional decline, social isolation, and emerging health risks—allowing providers and families to intervene before crises occur.

Independent studies show Zemplee’s systems can reduce hospital days and falls, and boost confidence for older adults and their families.

“Rural communities don’t need more reactive healthcare—they need proactive infrastructure,” said Aparna Pujar, Founder and CEO of Zemplee. “Being selected into AscendRural validates our belief that AI can serve as critical infrastructure for Medicaid, HCBS programs, and rural value-based care models. Our goal is simple: extend independence, reduce avoidable hospitalizations, and support caregivers—without adding complexity.”

Expanding Rural Partnerships

Through the AscendRural Accelerator, Zemplee will collaborate with rural health leaders, state agencies, and community-based providers to explore scalable deployments across rural markets.

Rural health systems, Medicaid agencies, PACE programs, and community-based organizations interested in advancing AI-enabled aging infrastructure are encouraged to connect.
